Health Partner, Inc. is the owner of the Our Health Partner website. This Privacy Policy describes the information that we collect on the site and the uses that we make of that information.

Collection of personally identifiable information (PII)

Registration

If you would like to use Health Partner, Inc.’s services, you will have to complete registration, which will involve having a username and password, and submit a credit check. This will allow us to provide our users with a customized payment plan and set up a direct link with their providers to make sure all payments and transactions proceed smoothly. This does include personal information, such as bank details, full name, address, procedure and treatment names, names of providers, email, and phone number (optional). We will need information such as employment status and some history, through the credit check. Once registered, you will login with your username and password and be assigned or given the option to choose a Health Partner payment plan.

Service

If you decide to sign up for a Health Partner payment plan, we request certain personal identifiable information (PII) from you. You must provide contact information and financial information (bank account or checking account) that we will use for billing purposes.  If we have trouble processing an order, we may use this information to contact you.  Although you will fill out the order form on our site, we use our bank, First Tennessee, to process the payment information.  Consequently, First Tennesse will also have access to your PII and credit card information.

Log Files and Cookies

We gather internet protocol (IP) addresses automatically and store it them in log files. We do not use cookies (small text files that are stored on a user’s computer for record-keeping purposes) on this site, nor are any advertiser cookies used.  Our site also makes no use of the tracking technology called clear gifs, sometimes known as web beacons or web bugs.

Use of Collected PII

General Use

We will use the customer’s PII to customize the customer’s payment plan, to facilitate communication among the customer, provider, and Health Partner,  and to process your application and order, as described above.  In addition, we will use your PII to communicate with you on other matters, as described below.

Patients and Providers

Only payment plan information will be shared between the patient and provider. All other patient information such as banking information and credit score will remain confidential to the patient and Health Partner. We will provide payment reports of the patient’s report to the provider.

Communications from the Site

Based upon the PII you provide us, we will send the user a welcoming email to verify your username and password.  We will also communicate with you in response to your inquiries, to provide the services you request, and to manage your account.  We will send you service-related announcements when it is necessary to do so and may also send you information on new programs that we develop.  If you do not want to receive information about new programs, please contact us as directed below and let us know.

Information Sharing and Disclosure

Third Parties

We do not share patient’s credit score information or payment information with any third parties.  As noted above, we use checking and other bank services to process payments, which does necessitate the need for bank account information.

Legal Disclaimer

We reserve the right to disclose your credit information as required by law and when we believe that disclosure is necessary to protect our rights and/or to comply with a judicial proceeding, court order, or legal process.

Access to Personally Identifiable Information

If your profile and PII changes, or if you no longer desire our services, you may correct, update, or delete your information by contacting us as directed below.

Security

The security of your PII is important to us. We have policies and procedures in place to protect your PII, both during transmission and once we receive it.  As we described above, when you enter your payment information on our site, First Data encrypts that information using secure socket layer technology (SSL).  While we consider these policies and procedures to be adequate, there is still a risk that your PII could be misappropriated.  No method of transmission over the Internet, or method of electronic storage, is completely secure.  Therefore, while we strive to use commercially acceptable means to protect your PII, we cannot guarantee its absolute security.  If you have any questions about security on our site, you can contact us as directed below.

Business Transitions

In the event Health Partner, Inc. goes through a business transition, such as a merger, acquisition by another company, or sale of all or a portion of its assets, we have the right to assign your PII to the other party to the transaction.  You will be notified via prominent notice on our site for 30 days of any such change in ownership or control of your PII.

Changes in this Privacy Statement

If we decide to change our privacy policy, we will post those changes to this Privacy Policy, our homepage, and such other places we deem appropriate so that you will be aware of any changes in what information we collect, how we use it, and under what circumstances, if any, we disclose it.  We reserve the right to modify this Privacy Policy at any time, so please review it frequently.  If we make material changes to this policy, we will notify you here and by means of a notice on our home page.
